Families wishing to formaly register as parishioners of St. Paschal Baylon Catholic Church may do so electronically right here using the simple forms below; or you can obtain a Parish Registration Form at the Parish Center.

Please complete just one Household Registration Form for the whole family, then one Member Registration Form for each member of your family (e.g. one for the head of household, one for the spouse and one for each child, including infants). Also, please let us know what ministries or activities you are participating in, or are interested in joining.
